New Patients & Insurance

BEFORE YOUR FIRST APPOINTMENT

Please download and complete the New Patient Packet in advance of your first appointment. You can email or fax the forms back or just bring them in with you. If you are unable to fill out the forms in advance, let us know and we will leave a set for you in the waiting room. Please arrive 15 minutes early to fill out.

Email an image of the front and back of your insurance card (please include Date Of Birth). This will speed up verification of benefits in advance of your first visit.

NEW PATIENT VISIT: what to expect

The initial New Patient visit includes consultation, examination, and treatment, and lasts about an hour.

Dr. Henderson is a Participating Provider in the following insurance networks

  • Carefirst BlueCross BlueShield, BlueChoice, Regional Preferred, and GHMSI Provider Networks (includes Federal Employee Program)

  • GEHA

  • Health Allies

  • Kaiser Mid-Atlantic

  • MAMSI

  • NexusACO

  • Signature Wellness

  • Stratos

  • United Health Care

While not a member of any other insurance networks, most non-HMO plans provide reimbursement for out-of-network chiropractic services, often after an out-of-network deductible is met. The office will assist in finding out what coverage is available and will submit claims electronically, but cannot guarantee that there will be insurance reimbursement. Payment is requested at time of service. All forms of paper and plastic are accepted.

MISSED APPOINTMENT FEE

A $60 charge will apply to all follow-up appointments that are either missed or canceled on the day of service. Please call or email (preferred) before close of business on the day before a scheduled appointment to avoid this charge.
