If you have scheduled your initial visit to this office, we are looking forward to meeting and working with you to address your health needs. As with any first-time visit to a doctor's office, there is some paperwork to take care of. All new patients must
-- fill out a confidential New Patient Questionnaire
-- Read the Privacy Policy and sign the Privacy Policy Consent Form
Currently the New Patient Packet is not downloadable. We will send you by email a .pdf version to fill out in advance to bring with you to your appointment. If by chance you don't receive the form within one or two days of scheduling your initial visit, please call the office at . Occasionally our human side slips out and we forget. As soon as we can have the downloadable form online again this page will change.
If you are unable to fill out the forms, in advance, please arrive about 20 minutes early for your first appointment. Please contact the office if you plan to do this so we can have it ready.
**A new patient appointment blocks off an hour of time on the office calendar. If you cannot make your appointment, please cancel before the close of business on the day before to free up the time for other patients. |
